Join our Support Coordination team and make a real positive difference to the lives of people livinjg with complex disabilities. This Support Coordinator role will provide quality support to NDIS participants and their families. We are seeking a passionate and experienced individual that will bring an element of enthusiasm and motivation to the already existing team. The successful candidate will play a crucial role in assisting NDIS participants to navigate and access the services they need to achieve their goals and build capacity in their everyday lives.

Key Responsibilities:
  • Coordinate and manage NDIS plans for participants.
  • Assist participants in understanding their NDIS plans and funding.
  • Connect participants with appropriate service providers.
  • Monitor and review participants’ needs and preferences.
  • Advocate for participants’ needs and preferences.
  • Maintain accurate and up-to-date records and documentation.
  • Collaborate with other team members and stakeholders to ensure comprehensive support.
  • Maintain team objectives and achieve reasonable Key Performance Indicators.

Requirements

To be successful in this role you will need to be empathetic and caring, be able to effectively communicate to a diverse range of people, have excellent verbal and written communication skills and organisational and time management skills. You will possess a strong acumen in engaging potential clients and onboarding them and have a proven record of developing and maintaining stakeholder relationships as well as an ability to work independently, and as part of a team and take initiative. You must also be open to construct feedback and have a positive attitude to personal and professional growth. You will have at least 1 years' experience in a support coordinator role, experience with complex psychosocial disability and a current drivers licence, and hold valid NDIS worker screening clearance and blue card.
