Description
CanaCare is a registered NDIS provider based in Byford, delivering specialized, participant-centred support services across the South of the River region in Perth. Led by an experienced Registered Nurse (RN), our diverse team combines high-level clinical expertise with lived experience to deliver holistic care. At CanaCare, we focus on building meaningful partnerships that empower participants to achieve their NDIS goals and live without limits.
Our services fall under the following NDIS support categories:
- Core Supports (Assistance with Daily Life & Social and Community Participation): High-intensity assistance with activities of daily living (ADLs), personal care, and supported community access to enhance independence.
- Capacity Building (Development of Daily Living and Life Skills): Specialized clinical training and support, including complex wound care, PEG feeding, stoma and catheter management, continence support, seizure management, brain injury support, and diabetes management.
We welcome the opportunity to collaborate with participants, families, and Support Coordinators to design tailored care plans that meet specific NDIS plan budget requirements.
