Description
My name is Phoebe Jo Bagnell, and I am the Founder and Support Coordinator of Compass Guidance & Supports, an independent disability support and Support Coordination service based on the Central Coast NSW.
We currently have immediate capacity with no waitlist for Support Connection (Level 1) and Coordination of Supports (Level 2) for plan-managed participants.
With 19 years of experience across disability and community services, I understand how important responsive communication, accurate documentation and proactive coordination are — not only for participants and families, but also for the providers and plan managers working alongside them.
At Compass, our approach includes:
• Prompt communication and onboarding
• Clear and accurate service agreements
• Proactive monitoring of participant budgets and plan utilisation
• Identifying under-utilised funding, service gaps and potential budget concerns early
• Supporting participants to prepare for plan reassessments and changes
• Working collaboratively with plan managers, allied health professionals and other providers
I strongly believe that good outcomes come from providers working with one another, rather than in isolation. We each bring different expertise, and building respectful relationships across a participant's support network can make an enormous difference.
