Description
Health On Call is a Registered NDIS Provider, founded by nurses and led by nurses, providing reliable, person-centred disability and nursing supports across Sydney.
With more than 20 years of combined healthcare experience, our team understands that every participant has different needs, goals, and preferences. We take the time to understand each person and carefully match them with suitable support workers and nurses.
Our services include Community Nursing Care, Complex and High-Intensity Nursing Supports, Personal Care, Community Access and Participation, Assistance with Daily Living, and Disability Support Work.
Our nursing team can support participants with complex health needs including wound care, medication support, diabetes management, continence care, catheter care, PEG/enteral feeding and other clinical nursing requirements within the practitioner's scope and participant's care plan.
We also support children and adults living with Autism, ADHD, intellectual disability, psychosocial disability and other support needs, with an emphasis on consistency, safety, dignity, independence and meaningful participation.
At Health On Call, we believe good support is not simply about filling a shift. We focus on finding the right person for the participant and building a reliable, long-term support relationship.

New referrals are welcome. Participants, families, Support Coordinators and Plan Managers are welcome to contact Health On Call to discuss individual support requirements.
